Theory of Mind
Theory of mind (ToM) is the cognitive capacity to attribute mental states — beliefs, desires, intentions, knowledge, and emotions — to oneself and to other people, and to understand that others' mental states may differ from one's own and from reality. It is the basis of everyday social cognition: when we wonder what a colleague thinks about a decision, anticipate how a friend will react to news, or recognise that a child is confused because they lack information we possess, we are deploying theory of mind.
The term was coined by David Premack and Guy Woodruff in 1978, in a paper asking whether a chimpanzee named Sarah possessed the ability to infer the goals and intentions behind human actions. The paper ended with a widely cited commentary section in which philosopher Daniel Dennett proposed the "false belief task" as the definitive test of true mental state attribution: to pass, a subject must represent that another agent holds a belief different from reality and predict their behaviour accordingly. Heinz Wimmer and Josef Perner (1983) implemented this proposal with their Maxi task, finding that children below age 4 consistently failed — apparently unable to represent the character's false belief independently of their own knowledge of where the object had moved.
Simon Baron-Cohen, Alan Leslie, and Uta Frith (1985) adapted the paradigm into the Sally-Anne task and applied it to children with autism. Their finding — that 80% of autistic children failed, versus only 15% of typically developing 4-year-olds and 14% of children with Down syndrome — was one of the most influential results in developmental psychology. The selectivity of the deficit (present in autism but not in general cognitive delay) supported the hypothesis of a specific theory of mind impairment in autism, which Baron-Cohen later elaborated as "mindblindness." Neuroimaging research has since implicated a dedicated "mentalising network" — centred on the medial prefrontal cortex, temporoparietal junction, and posterior superior temporal sulcus — in adult theory of mind. Robert Selman's (1980) five-stage developmental model of perspective-taking provides a complementary account of how the underlying social-cognitive capacities unfold across childhood and adolescence.

What is theory of mind?

In the Sally-Anne task (Baron-Cohen et al., 1985): Sally places a marble in a basket and leaves the room. Anne moves the marble to a box. The child is asked: "Where will Sally look for the marble?" A child with theory of mind answers "in the basket" — representing Sally's false belief. A child without ToM typically answers "in the box," reporting where the marble actually is rather than where Sally falsely believes it to be. Baron-Cohen et al. found that 80% of autistic children failed the task compared with about 15% of typically developing children aged 4.
When does theory of mind develop?
The standard false belief task is typically passed around age 4 in neurotypical children. Precursors emerge earlier: joint attention (sharing attention to an object with another person) develops around 9–12 months; pretend play by 18–24 months; and understanding of desires and simple intentions by age 2–3. More demanding second-order false belief tasks ("what does A think B thinks?") are typically passed around age 6–7. Some researchers using implicit, looking-time paradigms have suggested rudimentary ToM-like processing much earlier (infants under 2), though interpretation is contested.
Why is theory of mind said to be impaired in autism?
Baron-Cohen et al. (1985) found that most autistic children failed the Sally-Anne false belief task despite being cognitively old enough to pass. The deficit was specific to autism — not seen in children with Down syndrome (who have general cognitive delay). Baron-Cohen (1995) proposed "mindblindness" as the core feature: difficulty automatically mentalising — representing what others know, believe, and feel. This makes social interaction unpredictable and effortful. However, the picture has become more nuanced: many autistic adults pass standard false belief tasks but report social difficulties in more complex real-world situations, suggesting compensatory strategies or a distinction between explicit and implicit mentalising.
