Prefrontal Cortex

The prefrontal cortex (PFC) is the anterior portion of the frontal lobe — the cortex at the very front of the brain. It constitutes approximately one-third of the human cerebral cortex and is disproportionately large relative to other species, reflecting its central role in the cognitive abilities that characterise human behaviour. The PFC is the primary neural substrate for executive functions: higher-order processes involved in planning, goal-directed behaviour, and the flexible regulation of thought and action.

These executive functions include working memory (holding information in mind while using it), inhibitory control (suppressing inappropriate or impulsive responses), cognitive flexibility (shifting between tasks or mental sets), planning, and decision-making. The PFC also governs social behaviour, empathy, and emotional regulation — integrating information about context, past experience, and anticipated consequences. The case of Phineas Gage — a railroad foreman who survived a tamping iron passing through his prefrontal cortex in 1848 — provided the first systematic evidence of this role. While he retained language, memory, and basic intelligence, his personality changed dramatically: his physician John Harlow recorded that he had become fitful, irreverent, and unable to adhere to plans or social conventions.

The PFC undergoes a prolonged developmental trajectory, with myelination and synaptic pruning continuing into the mid-20s. This late maturation has important implications for adolescent behaviour: the limbic system — driving reward-seeking and emotional reactivity — matures earlier than prefrontal regulatory systems, producing the characteristic risk-taking and peer-sensitivity of adolescence.
